When you arrive
When you arrive, speak with the receptionist and make yourself comfortable in our waiting room.
Please note that I do not accept payment from insurance companies. I will provide you with a receipt for the visit along with my input on the relevant insurance form so that you can submit them to your insurance company for reimbursement. Please check with your insurance company regarding their policies on this. Questions to ask include:
- Does my plan cover mental health benefits?
- Does my plan cover psychiatric medication?
- How many sessions are covered?
- Does my plan cover laboratory procedures?
- What is the reimbursement rate?
- If necessary, I can assist you in negotiating with your insurance company.
If you wish to claim back from your insurance company for your visit please remember to collect a form from the receptionist and bring with you to the doctor.
Please pay the required fee by cash or check at the end of each session, the fee per session. All cheques are to be made payable to Dr. E. Anthony Allen.

Persons or couples/families who will be engaging in a course of psychotherapy sessions or in long-term care, and who will have difficulty maintaining the stated fees, should discuss this. Efforts will be made to arrive at an agreement with regard to concessions which would allow the desired therapy.
The usual appointment is for forty (40) minutes. Sometimes there are overruns which cannot be predicted. Thus I provide for some flexibility. Though I try to avoid patients waiting unduly, I ask for your patience if this occurs, due to unexpected circumstances or unavoidable overruns.
Couples or families who wish to book an appointment for 1 hour could arrange this with the receptionist.
Those who wish regular psychotherapy or counseling appointments 2 to 3 weeks apart, are asked to book at least 2 appointments ahead of time.
You are being asked to confirm your appointments two days ahead of time. This is necessary as sometimes patients fail to attend without due notice. This makes it difficult for other patients who could be seen earlier but end up having their appointments delayed unnecessarily.
You are being asked to pay 50% for all scheduled sessions you have missed unless you cancel at least 24 hours before your scheduled appointment. Exceptions are made only in an emergency, at my discretion. Your not turning up without notice will deprive other patients of service..
MAKING FOLLOW UP APPOINTMENTS
Please book your next appointment as soon as possible after your last appointment and make sure you return before your prescription is finished. Otherwise have the prescription repeated by your General Practitioner until your next appointment.
Except for when unavoidable situations necessitate these, repeat prescriptions will attract a fee of $700.00. These will be written only under circumstances beyond the patient’s control. Call if in doubt.
After your first appointment please feel free to call me with regards to:
- Distressing side effects of your medication(s)
- Worsening of your symptoms
- Any other necessary and urgent inquiries.
If I am not available please leave a message with the receptionist or on voicemail and I will return your call.
Please arrange to have a thorough physical examination within a few weeks of therapy, if you have not had one within the last 12 months. Kindly provide me with a report of the findings.
Please make it a point to know the name, strength and dosage of all the medications you are taking from myself or any other physician. This will be necessary at those times when you are being seen or are making telephone enquiries
You will be given a form to allow you to evaluate our services after your first two visits We appreciate your completing this as it will assist us to improve our services. Please feel free to leave your suggestions in the suggestion box. This will help us to serve you better.

Handouts are available on various conditions and life situations which might apply to you, and also on personal and family growth. Books on certain issues are also available for purchase. Please check the receptionist for handouts or books on any topic of your choice.
At Durham Avenue, DVD’s and videotapes are available for viewing in the waiting room. This website advertises several articles, book and video DVD’s that can be ordered instantly. There are also several links to educational topics. You may also use the search function for other topics of your choice.
